The thing that does not get talked about during this pandemic is that it's been exacerbated in the US by the deliberate reduction in hospital beds during the neoliberal project.
The US used to have a pretty good, affordable public hospital system (which is why we got away without single payer for so long), and then we fucking sold them off for pennies on the dollar, and they were asset-stripped by corporations, especially in rural areas and the inner-city, where they weren't profitable.

Instance suspension doesn't have to be a permanent thing. I have unsuspended instances in the past when they've publicly owned their problems and taken visible action to correct them.
As a federated community, this is really the only punitive measure available to us at an instance level; it's time we started treating it as less of a monolith and instead treat it like the community moderation tool it is.

I think of this issue as being akin to a hostage situation. Birdsite & FB also had famous bad actors unabashedly breaking their rules, but they are beholden to those bad actors. They can't make hard decisions without potentially going out of business.
We can, and thus IMHO have a responsibility to do so if we want to maintain any semblance of control over our communities.
