My intake forms for SRS mention I should send a "picture of [my]self" along with the forms. The same surgeon does minor facial surgery. That's gotta be for that, right? I just feel weird including a selfie with a document about below-the-belt surgery.

@ikea_femme Is the picture of self meant to be your face? (sorry if that's weird to suggest, but I feel like "self" is pretty encompassing?)
